‘SWIIS Healthcare Limited’ is a specialist healthcare and social care company, providing a range of high quality staffing and training solutions to the health and social care sectors including; PCT’s, NHS, private hospitals, community nursing, housing, charities, voluntary sectors and drug and alcohol units. We place both qualified and unqualified staff of an exceptional standard, all of whom are provided with 24 hour management support
We are currently seeking a number of experienced staff to work within a number of floating support schemes in the Bristol area.
Applications are welcomed from people who have worked in similar services before and have knowledge and experience of the following:
• Manage a caseload of customers who have generic / multiple needs and are living in independent accommodation or who are at risk of becoming homeless.
• Assess the support needs of people referred to the service, maintain and review support plans.
• Provide practical and emotional 'housing related support' and advice as necessary to enable the client to manage / obtain their accommodation.
• Refer customers to appropriate statutory and voluntary agencies, negotiating service delivery and advocating on behalf of customers to ensure adequate services are provided.
• Assisting service users to develop life skills and partake in meaningful occupation activities to promote increased independence and involvement in the community.
Experience:
Candidates should possess (or working towards) a min of NVQ Level 2.
At least two year`s recent experience of working with a floating support service
Experience of working in community based residential services, supported housing or outreach services.
